Rapids Family Health Team sets Nov. 2 opening for preventive care clinic
The new space at 1150 Pontiac Drive will house nurse educators and offer diabetes education, women’s health and menopause care, and support for people with chronic conditions.

Rapids Family Health Team has scheduled a Nov. 2 grand opening for a new preventive care clinic at 1150 Pontiac Dr. in Sarnia.
The 3,000-square-foot space is in the same building as the team’s main office, two doors down, according to an Oct. 29 release.
Rapids says the clinic will house its nurse educators and provide space for disease prevention, chronic disease management and health education. Planned services include diabetes education, women’s health and menopause care, and support for people living with chronic conditions.
The team says the dedicated space is intended to give patients more room for education, support and resources.
Information about programs and services is available through Rapids Family Health Team or by calling 519-339-8949.
